I think I lost mine because I would get so many notifications (new replies, new topics, edits) each day that some of the notifications would get pushed past the tab and become hidden, no longer displaying as a notification.
Like this:
No notification dots, and no unread tiles.
But,
So yeah, due to not knowing how many unread things I have, I too lost my regular status.

I have notifications for everything turned on.
