There is a bug with heroes that have second lives. When they revive, they instantly get ko’ed. I took a close look nobody took damage after they revive so this definitely a bug.
This has already been brought up to PerBlue in the Known Issues thread. Please check there to see if a bug has already been brought to their attention before posting about it next time.
